At any time for EMERGENCIES

Whatever the day or time, if you or someone else experiences severe chest pain, loss of blood or suspected broken bones, go to your nearest accident and emergency department
or call 999.

What is FAST?

FAST requires an assessment of three specific symptoms of stroke:

  1. Facial weakness - can the person smile? Has their mouth or eye drooped?
  2. Arm weakness - can the person raise both arms
  3. Speech problems - can the person speak clearly and understand what you say?
  4. Time to call 999

 

A Brief Introduction to the Practice

Welcome to our Practice website.  We aim to offer a full range of health services to you and your family, in a friendly and efficient environment.  If you wish to register please ask the receptionist for the relevant forms. 

We have produced this website to provide you with information about the way the Practice is organised and the services that are provided directly by Practice Staff, or by other members of the Primary Health Care Team.  Services will, by necessity, change from time to time, please do not hesitate to telephone the Practice to check the current position.

Newly Registered Patients

To register with our practice, please download our Registration Forms, or call in to the Surgery and collect them from reception, completing these froms enables the Practice to manage ongoing medical conditions.

Our Registration Forms (Patient and Childrens) can be downloaded as Word documents for completion on your personal computer, or on pdf format to print and complete by hand.

Please ensure you bring with you one of the following as proof of ID:

  1. Passport
  2. Driving License
  3. ID Card

We also require one of the following as proof of address:

  1. Tenancy Agreement
  2. Utility Bill
  3. Bank or Credit Card Statement

You will be registered with the Practice as a whole and not with an individual Doctor, but you are free to consult any of the Doctors within the Practice.  It is usually best to keep to the same Doctor during any particular illness.

Temporary Residents

We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse.  They will need to fill out a short form for the information to be returned to their own Doctor.  Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.

Children and the Waiting Room

We have a small play area in the waiting room which is unsupervised.  We do try to keep a selection of toys available which all are welcome to use, but we do invite parents to bring (and take away again!) toys to amuse their own children.

Monday Tuesday and Friday
8:00am - 6:30pm
Wednesday
8:00am - 1:30pm
Thursday
8:00am - 7:30pm

Emergencies will be seen immediately by the Doctor or Nurse Practitioner, who will make a preliminary assessment.  If your appointment is for anything other than a normal consultation, e.g. is for cervical smear or for medical examination, please inform the Receptionist at the time of booking.

Parking and Access for the Disabled

Street parking is available on the side street opposite (Baronsmead Road) There is a rear entrance to the surgery for wheelchair users (with prior notice) and there is limited space for prams and pushchairs in the communal hallway and porch.
